All team players, team representatives and league officials must register for the 2024/2025 SMVA volleyball season
All information entered on this form must be accurate and up to date

Member/Team Responsibilities

Each member will read and abide by the SMVA house rules and Team Responsibilities
All communications between SMVA league teams and the SMVA executive will be done through the team captain or assistant captains
The SMVA executive has the right to discipline, suspend or revoke membership to any member, as it deems fit, in any capacity

WAIVER - Membership can be lost	I understand and agree that the SMVA executive has the right to discipline, suspend or revoke membership to any member, as it deems fit, in any capacity.
Waiver	I hereby certify that as a member of the SMVA and/or a participant in the volleyball league in whatever capacity, I understand there is an assumption of risk on my part as a willing participant. Volleyball is a sport and there is an chance of Injury. I release the SMVA, members of the executive, its successors, officers, agents and all employees and volunteers and other members in the league, from any claims, demands, and cause of action whatsoever in any way growing out of or resulting from participation in the activities listed above. I have read the terms above and agree.
Understand this is a waiver!	Yes, I understand that this is a waiver by signing this document I will waive certain legal rights, INCLUDING THE RIGHT TO SUE or claim compensation following an accident.
Waiver agreement	PLEASE TAKE YOUR TIME AND READ CAREFULLY. I HAVE READ THIS RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT, AND SIGN IF FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT.
